2 killed in BASE jumping incident in Utah, including Two people were killed in a BASE jumping incident in Utah, including an extreme athlete who performed with Madonna during the 2012 Super Bowl, authorities said. The incident occurred Sunday in a remote area of Mineral Bottom in Grand County, according to the local sheriff's office. Deputies from the Grand County Sheriff's Office, emergency medical services personnel and two helicopters responded, authorities said. Two men died from their injuries at the scene, according to the Grand County Sheriff's Office -- an approximately 50-year-old man, whose name was not released, and Andrew "Andy" Lewis of Moab.
